Please be aware that most forms are done “on-line”, either on this website , or they are done on the Rank One website . Physical form is a “paper” form, you need print it out, fill and sign it, and remember to bring it to Registration.

A parent should complete this form (you’ll need the Student ID number). Directory information for up to 3 students from the same family can be placed on one form.

Physical Form

REQUIRED for ALL Band & Vision Students

All students must have a physical this year.  Get this completed and signed by your physician.  Bring the completed form to Registration to turn in on July 25, 2026.  Click on the section title above (“Physical Form”) to download the blank paper form, or you can download the same form from the Rank One website.

There are MULTIPLE forms on the RankOne website for Band & Vision Students

A parent must complete the forms with the student.  Click the section title above to go to the RankOne website.  The RankOne website is scheduled to go live this year on July 1st, 2026.  DO NOT fill the RankOne forms out before July 1st, otherwise you will have to fill them out again.  Here are the forms that are on RankOne:

  • Extracurricular Handbook(s) Acknowledgement Form – REQUIRED for ALL Band and Vision
  • LISD Instrument Loan Agreement – REQUIRED if you are renting your instrument from LISD
  • LISD Photo Release – REQUIRED for ALL Band and Vision
  • Travel and OTC Medication Consent – REQUIRED for ALL Band and Vision
  • Trip Authorization – REQUIRED for ALL Band and Vision
  • Emergency Card – REQUIRED for ALL Band and Vision
  • UIL Marching Band Acknowledgment Form – REQUIRED for ALL Band and Vision

Mentoring

Click on this link to take you to the Mentoring Interest form.  Here you can sign up to be a mentor, or you can request a mentor.  Mentoring is a great way to give back, and it can be extremely helpful for “New to Program” parents – there is a LOT to take in.
